seeking a passionate and experienced *Care Manager* to join our
team at the detox and residential level of care for adults.

As a *Care Manager*, you will support clients through every phase
of their treatment journey, including care coordination, aftercare
planning, case management, and discharge preparation. You will play

*Care Manager Requirements*
* Bachelor's degree in a related field preferred (required for
detox level of care); high school diploma required at minimum
* Minimum of one year of experience working in the substance use
disorder field
* Knowledge of addiction, mental health, treatment resources, and
12-Step recovery
* Familiarity with discharge planning, aftercare coordination, and
case management
* CPR certification preferred
* CRSS, CAC, CAP, LCSW, LMHC, or Registered Intern (preferred,
willingness to obtain a plus)
*Care Manager Responsibilities*
* Coordinate discharge planning and assist clients with transitions
to aftercare programs
* Communicate with outside providers, referents, and family members
(with proper releases in place)
* Facilitate individual case management sessions and maintain
ongoing communication with clients
* Participate in intervention strategies for clients at risk of
leaving treatment Against Medical Advice (AMA)
* Monitor client participation, attendance, and adherence to the
daily treatment schedule
* Assist with clinical documentation, including assessments and
treatment planning, as appropriate
* Facilitate weekly clinical groups and participate in recreational
and weekend activities as needed
* Maintain accurate and timely documentation in accordance with
company standards
* Coordinate and assist with client transportation as needed
* Assist clients with daily living essentials and ancillary
financial accounts
* Monitor and support agency drug screening processes under
supervision
* Uphold patient privacy and confidentiality in accordance with
HIPAA and 42 CFR regulations
* Report concerns about client privacy, safety, or quality of care
to appropriate leadership without fear of retaliation
* Support and enforce facility policies, procedures, and program
expectations
* Perform other duties as assigned
*Pay:* $40,000-60,000
*Schedule:* Full-time
